What to expect from me [Cindy Woolley RD] as a Health Care Professional, and what Cindy Woolley RD will expect from you as a client.
Cindy Woolley RD is covered by the Health Care Professionals Council (HCPC) and adheres to strict standards of proficiency (see https://www.hcpc-uk.org/globalassets/resources/standards/standards-of-proficiency---dietitians.pdf for further details). Written information of these standards can also be provided upon request.
Dietetic advice provided by Cindy Woolley RD will always be based on the most up to date evidence and guidelines (see BDA for policy https://www.bda.uk.com/uploads/assets/ef8656c5-320e-4d8b5c7ff7c82519d47/Code-of-Conduct.pdf
Cindy Woolley RD cannot guarantee any existing health conditions will be cured or improved after dietetic intervention.
Weight loss cannot be guaranteed by Cindy Woolley RD as part of the weight loss programme, as many factors can influence this.
Dietary advice provided by Cindy Woolley RD will be based on medical history provided by the client prior to the appointment, or via their GP (see Client Health Questionnaire). Clients have a responsibility to advise Cindy Woolley RD of any changes to medical history during dietetic intervention, as this may affect dietary advice provided.
A dietetic appointment will not be confirmed until initial Client Health Questionnaire has been returned, and medical history reviewed. If for any reason I am unable to proceed with a consultation, after reviewing the health questionnaire, the client will be contacted and advise of reason for this decision.
Cindy Woolley RD will require return of a 3-day food diary at least 48 hours prior to a booked appointment, to allow an accurate dietary assessment to be completed. If this is not returned, then the consultation may not be able to go ahead and appointment will need to be rebooked.
Cindy Woolley RD will provide at least 24 hours notice if a booked appointment needs to be cancelled for any reason.
Cindy Woolley RD will attempt to reply to any emails, telephone calls etc within a 48-hour time period.
The following payment methods will be accepted:
BACS: Full payment to be made at least 24 hours prior to appointment. (Santander A/c number: 76223182 Sort code 09-01-35)
I regret that cheques will not be an acceptable form of payment. If you are unable to pay via bank transfer, please contact me to discuss alternative options.
Home visits will incur a small additional mileage fee, if travel distance exceeds a 5 miles radius from my office location in LE7 2HB (0.56p/mile)
Cancellation / Fail to Attend
Cancellation of any consultation (clinic, telephone or online) needs to be made at least 24 hrs prior to appointment. This can be done either by text, email or telephone.
If cancellation is not made within 24 hrs, or you fail to attend the appointment (clinic, telephone or online), an administration fee of £20 will be incurred and taken from monies already paid prior to appointment
Prompt arrival is required for all appointments. If you are more than 5 mins late your appointment slot cannot be guaranteed, due to the impact on other client appointments, and may need to be rescheduled.
By making payment, you confirm that you have read this document and agree to the T & C’s outlined above
